What data is collected?
The personal data I would hold about you may include:
- Your name, yard address, email address, phone number
- The initial New Client form which you have completed on the first session
- Detailed notes from each session
- Any other information relevant to our sessions which you have wanted to share
Why do I need to hold your personal data?
- In order to be able to communicate with you via phone, email or post.
- So that I have a record of my attendance all our sessions
- It is a requirement of my Insurance provider
How long will your data be kept?
- My Insurance provider requires me to keep an client records for at least 5 years after completing the ET sessions
- After this 5 years, all electronic data can be permanently deleted and any hard copies shredded and disposed.
Your right to see the data
- You have the right to see the personal data I am holding on you.
- If you wish to do so, you can ask to see this data, and I will be required to provide this for you within 40 days of your request
